For Filipinos Contracting Marriage in Kuwait

  1. Certificate of No Marriage Record (CENOMAR) obtained from the Philippine Statistics Authority (PSA) in the Philippines and duly Apostillized by the Department of Foreign Affairs (DFA) in Manila
  2. Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) and duly Apostillized by the Department of Foreign Affairs (DFA) in Manila
  3. Submit both documents to the Philippine Embassy in Kuwait for issuance of a Certificate of Legal Capacity to Contract Marriage (LCCM).
  4. Sworn Statement stating that he/she is single (if sending the documents by post, the Sworn Statement must be notarized by a local notary public and Apostillized by a competent authority in the issuing country)
  5. Payment of fees: KD 8.125 for LCCM and KD 8.125 for the Sworn Statement

 

The death of a Filipino in Kuwait should be reported and registered with the Philippine Statistics Authority through the Philippine Embassy. The Report of Death (ROD) should be done within 12 months of the date of death.
 
The Embassy issues Consular Mortuary Certificates when remain of a deceased Filipino or foreigner are to be transported to the Philippines.
 
All documents not in English must be accompanied by an English translation.
 

Requirements

  1. Duly accomplished Report of Death Form bearing the original signature of the informant - bring four (4) signed originals
  2. Death Report of the deceased, Issued by a competent authority (Hospitals, and Forensic Department) in the country. 
  3. Passport and Civil ID of deceased - four (4) photocopies of data page (page showing photo and complete name)
  4. Affidavit of Delayed Registration of Death if the Report of Death is filed more than one (1) year after demise of the deceased - one (1) signed original and three (3) photocopies
  5. Payment of fees (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
    • Report of Death - KD 8.125
    • Affidavit - KD 8.125 per affidavit

 

Additional requirements for the shipment of human remains to the Philippines

All below must be original and Apostillized by a competent authority in the country where the deceased passed away; these are in addition to nos. 1-6 above.

  1. Coroner's Certificate of the Fact of Death or in its absence, an interim Death Certificate issued by the Coroner's Office
  2. Embalming Certificate issued by the funeral parlor
  3. Certificate of Sealing of Casket
  4. Certificate of Non-Contagious Disease issued by the local health authority that contains the following information a statement that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from place contaminated by a contagious disease
  5. Laissez-Passer for a Corpse/Removal Certificate issued by the local authorities
  6. Full repatriation/flight details
  7. Consignee's complete contact details (address, phone numbers, and email addresses)
  8. Payment of fees (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
    • Consular Mortuary Certificate (to be issued by Embassy after submission of complete requirements) - KD 8.125
    • Expedite fee – KD 3.250

If the remains are not of a Filipino national, then the requirements for Report of Death are not needed.

 

Additional requirements for the shipment of an urn to the Philippines

All below must be original and Apostillized by a competent authority in the country where the deceased passed away; these are in addition to nos. 1-6 above.

  1. Certificate issued by the mortuary/funeral director that contains the following information: (a) Statement that the body was properly cremated in accordance with internationally accepted health practices; (b) Statement that the body is in a sanitary condition shipment abroad; (c) Statement that the urn has been hermetically sealed and contains only the remains of the deceased
  2. Certificate of Non-Contagious Disease issued by the local health authority that contains the following information a statement that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from place contaminated by a contagious disease
  3. Transit Permit issued by the local authority at the place where transportation begins
  4. Copy of complete flight details/airway bill
  5. Consignee's complete contact details (address, phone numbers, and email addresses)
  6. Payment of fees (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
    • Consular Mortuary Certificate (to be issued by Embassy after submission of complete requirements) - KD 8.125
    • Expedite fee for same-day processing (only for those submitting requirements in person at the Embassy) - KD 3.250

If the remains are not of a Filipino national, then the requirements for Report of Death are not needed.

Applicants will receive instructions after their requirements are received by the Embassy and are found to be complete - please include your contact details with your documents. All payments made to the Embassy are non-refundable.

Processing time is three (3) working days upon receipt of complete documents and payment.

The Embassy will forward the Report of Death to the Philippine Statistics Authority (PSA) in the Philippines. Another copy will be kept as part of the Embassy’s records and the a copy will be given to the applicant. You may retrieve the PSA-certified Report of Death from the Philippines three (3) to six (6) months after reporting the death at the Embassy.

Presentation of the official receipt issued by the Embassy is required if claiming your personal copy of the Report of Death in person at the Embassy.

  • For all fields requiring dates, please use the dd-MMM-yyyy format (e.g. 23-FEB-2012, 1-NOV-1984).
  • Please do not forget to affix by hand the signature in the field provided for Signature of Informant and the date and place of signature at the bottom of the form.
  • If filling out the form by hand, please write legibly and please use BLOCK CAPITAL letters.
 
 
Marriages abroad of Filipino citizens must be reported and duly registered. The Philippine Embassy registers marriages of Filipino citizens celebrated in Kuwait.
 
All documents not in English must be accompanied by an English translation.
 

Requirements

  1. Personal appearance of the persons executing the document at the Embassy from Sunday to Thursday, 08:00 AM to 12:00 NN
  2. Duly accomplished Report of Marriage form - four (4) signed originals
  3. Foreign marriage certificate of the applicants, Apostillized by a competent authority in the country of marriage - one (1) original and four (4) photocopies; original will be returned to applicants (Note: If the document is not in English, kindly have the document translated into English and have the translation notarized, before having it Apostillized.) 
  4. Passports of husband and wife - original and four (4) photocopies of each passport's data page (page showing photo and complete name).  Also, please bring copies of civil id of husband and wife.
  5. Birth certificates of husband and wife - original and four (4) photocopies each; original will be returned to applicant (for Filipino spouse, Birth Certificate must be issued by the Philippine Statistics Authority (PSA)
  6. Affidavit of Delayed Registration of Marriage if the Report of Marriage is filed more than one (1) year after applicants' date of marriage - four (4) signed originals
  7. For divorced or annulled Filipino spouseMarriage Certificate issued by the PSA with proper annotation - original and four (4) photocopies
  8. For divorced Filipino spouse who has no marriage record in the PSADivorce Decree/Certificate validated by the Regional Trial Court nearest the spouse’s place of residence in the Philippines - original and four (4) photocopies; original will be returned to applicant
  9. For widowed Filipino spouse, Death Certificate issued by PSA or the foreign death certificate of previous spouse and Marriage Certificate issued by PSA with the previous spouse - original and four (4) photocopies each; original will be returned to applicant
  10. For divorced foreign spouseforeign divorce decree - original and four (4) photocopies; original will be returned to applicant
  11. For widowed foreign spousedeath certificate of previous spouse - original and four (4) photocopies; original will be returned to applicant
  12. Payment of fees (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
    • Report of Marriage – KD 8.125
    • Affidavit – KD 8.125 per document
    • Expedite fee for same-day processing (only for those submitting requirements in person at the Embassy) – KD 3.250
  13. The Consular Officer reserves the right to request additional documents from the applicant.

You may request your PSA-certified Report of Marriage directly from PSA three (3) to six (6) months after reporting the marriage at the Embassy - the Embassy does not issue PSA documents.

Births of Filipino citizens abroad must be reported and duly registered. The Philippine Embassy registers births of Filipino citizens in Kuwait.
 
All documents not in English must be accompanied by an English translation.

  4. Passports of parents - four (4) photocopies of each passport's data page (page showing photo and complete name)
  5. Civil ID of Parents (if applicable)
  6. Parents' Marriage Certificate (MC) or Report of Marriage in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) - original and four (4) photocopies; original will be returned to applicant (If Marriage Certificate or Report of Marriage is not in English, please translate in English).
  7. Affidavit of Delayed Registration of Birth if the Report of Birth is filed more than one (1) year after applicant's birth date - four (4) signed originals
  8. Affidavit to Use the Surname of the Father executed by the mother, if parents are not married and the parents would like the applicant to bear the father's surname (if not filed, the applicant will automatically bear the surname of the mother) - four (4) signed originals
  9. Affidavit of Admission of Paternity executed by the father, if parents are not married and the parents would like the father to appear on the birth record and/or the applicant to bear the father's surname (if not filed, the applicant will automatically bear the surname of the mother) - four (4) signed originals
  10. Certificate of Registration of affidavits no. 6 and no. 7, if parents are not married - to be provided by the Embassy
  11. Payment of fees (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
    • Report of Birth - KD 8.125
    • Affidavit – KD 8.125 per affidavit
    • Certificate of Registration – KD 8.125
    • Expedite fee for same-day processing – KD 3.250
