Applicant must present the original with a photocopy of the following valid identification:

Under Republic Act No. 8239, also known as the "Philippine Passport Act of 1996," a Philippine passport is a document issued by the Philippine government to its citizens requesting other governments to allow its citizens to pass freely, and in case of need to give them lawful aid and protection [RA 8239, Sec. (d)]. The acquisition of a Philippine passport is a privilege. The law recognizes the passport applicant’s constitutional right to travel. However, the State is also mandated to protect and maintain the integrity and credibility of the passport and travel documents proceeding from it. It is a function of our Philippine consular officers to pass upon claims for or resolve matters involving Philippine citizenship and, upon determination of such citizenship, to issue an applicant with a Philippine passport. The passport remains a property of the Philippine Government and should be surrendered to the State should it be demanded through a competent Philippine Government Official.
Filipino citizens are entitled to a Philippine passport. Filipino citizens who have acquired foreign citizenship are eligible to apply for a Philippine passport after reacquisition of their Philippine citizenship (Oath of Allegiance under RA 9225).

BASIC REQUIREMENTS FOR ADULT RENEWAL (ELECTRONIC PASSPORT) APPLICATION
- Confirmed Online Appointment;
- Accomplished Application Form
- Personal Appearance
- Current ePassport with photocopy of data page; and
- Original PSA-issued documents that will support the change of name.
- Certificate of Marriage if married women will use spouse's surname;
- Annotated Certificate of Live Birth if with corrected entries;
- Annotated Certificate of Marriage to show annulment/divorce/court ordered instruction; and/or
- Certificate of Death of Spouse if married women will revert to the use of maiden name.
- Payment of fees: KD 19.500 (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
Note: If a woman opts to retain maiden name, a PSA-issued Certificate of Marriage is not required.
Supporting Documents for Adult Renewal of ePassport Applications
Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:

For Lost VALID and EXPIRED Passports
- Copy lost Passport (if available);
- Police Report with English translation;
- Notarized Affidavit of Loss; and

For Renewal of a frequently Lost Passport
- Police Report with English translation;
- Notarized Affidavit of Loss; and
- Amount to be paid: KWD 48.750.

For applicants with Unclaimed Passports
Applicant may request for a Certificate of Unclaimed Passport either at the office where the unclaimed passport was issued, nearest consular office or at DFA Aseana; and
- Certificate of Unclaimed Passport; and
- Notarized Affidavit of Explanation.
Additional supporting documents may be required as necessary to ascertain citizenship, identity and lack of travel restrictions.
In case applicant presented a one-way Travel Document/Travel Certificate for Passport Application
- Original Travel Document; and
- Notarized Affidavit of Explanation.

In case applicant Lost the one-way Latest-Issued Travel Document/Travel Certificate
- Travel Records from Bureau of Immigration or Travel History or records of arrival from the immigration authority of the host country;
- PSA-issued Certificate of Live Birth or PSA Repot of Birth;
- Any of the following valid identification cards with one (1) photocopy; and
- Notarized Affidavit of Explanation.

For Damaged or Mutilated Passport
- Notarized Affidavit of Explanation;
- PSA-issued Certificate of Live Birth;
- Any of the following valid identification cards with one (1) photocopy; and
- Amount to be paid: KWD 48.750.
For Dual Citizen Passport Applicants
- Natural-born Dual Citizens:
- Latest-issued Philippine passport; and
- PSA-issued Certificate of Live Birth or PSA-issued Report of Birth.
- Dual Citizens who availed of the provisions of Republic Act No. 9225:
- Latest-issued Philippine passport;
- Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine FSP; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by BI; and
- Valid government-issued ID or their secondary citizenship/residence counterparts.
For Naturalized Citizen Passport Applicants
- Latest-issued Philippine passport; and
- Identification Certificate of Naturalization from the Bureau of Immigration, or Certificate of Naturalization issued by the Special Committee on Naturalization (SCN), or copy of the final Court Judgment or Law granting Philippine citizenship.
For Applicants who are Filipino Citizens by Election
- Latest-issued Philippine passport;
- Documents of election of Philippine citizenship; and
- Identification Certificate issued by the BI.
If a Married Woman Applicant Wishes to Revert to her Maiden Name
- By virtue of death of the spouse:
- PSA-issued Certificate of Death or Report of Death (ROD) of spouse or apostillized or authenticated Foreign Death Certificate of foreign spouse with English translation, if applicable;
- PSA-issued Certificate of Live Birth or Report of Birth; and
- Latest issued Philippine passport (if available).
- By virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
- PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
- PSA-issued Certificate of Live Birth or Report of Birth; and
- Latest issued Philippine passport (if available).
- For other reasons of reversion:
- PSA-issued Certificate of Live Birth or Report of Birth;
- Notarized Affidavit of Explanation that includes request for the reversion of maiden name in the Philippine passport or travel document and stating she has not hitherto availed of the reversion
- Latest-issued Philippine passport or travel document; and
- Any existing government-issued valid ID accepted for passport application reflecting the maiden name.
Note: This mode of reversion can only be done once.

Supporting Documents for Adult New Applications
Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:
For Late Registered PSA-issued Certificate of Live Birth, Report of Birth, or Certificate of Foundling
- Original and photocopy of PSA-issued Certificate of Live Birth, Report of Birth, or Certificate of Foundling;
- At least one of the following on top of the basic requirements:
- Additional primary government-issued valid ID accepted for passport application;
- If unable to provide an additional primary ID, the applicant must submit any two (2) of the following documents:
- NBI Clearance (valid or expired);
- Voter's Certification;
- School Records such as:
- Form 137-A
- Transcript of Records
- Diploma obtained from Elementary, High School and/or College
- If government employee:
- Service Record;
- Member Data Record (MDR) from Philhealth
If the passport applicant’s PSA-issued Certificate of Live Birth, Report of Birth, or Certificate of Foundling contains a misspelled first or last name, or misspelled birth place, or mistake in the day or month of birth, or clerical error in the sex, or change of first name or nickname:
- Applicant must submit a PSA-annotated Certificate of Live Birth, Report of Birth, or Certificate of Foundling pursuant to R.A. No. 9048, as amended by R.A. No. 10172.
If by operation of law or through a court order, the passport applicant is permitted to use a name or other biographic detail other than what is officially recorded in the PSA:
- Annotated Certificate of Live Birth, Report of Birth, or Certificate of Foundling as authenticated by PSA reflecting the corrected entry.
Discrepancies on the biographical details of supporting documents submitted for passport application:
The applicant shall provide supporting documents that are consistent with the information as stated in the applicant’s PSA-issued documents, unless by operation of law or through court order, the applicant is permitted to use a name other than what is officially recorded in the PSA.
Thus, the applicant shall have the supporting document corrected in order for its details to be consistent with the PSA-issued document.
The applicant shall submit the following, depending on the correction needed:
- Annotated PSA Certificate of Live Birth
- Annotated PSA Marriage Certificate
- Corrected Government-issued Valid IDs
- Corrected Identification Certification
Additional supporting documents if the PSA-issued document is unreadable:
- Unreadable PSA-issued Certificate of Live Birth (COLB) or Certificate of Foundling:
- Local Civil Registry Municipal Form No. 102 or Civil Registry Form 1-A;
- Unreadable PSA-issued Certificate of Marriage:
- Local Civil Registry Municipal Form No. 97
- Unreadable PSA-issued Certificate of Death:
- Local Civil Registry Municipal Form No. 103
In case applicant has NO PSA-issued Certificate of Live Birth or Report of Birth:
- Applicant must first file for late registration with Local Civil Registrar (LCR) or Foreign Service Post (FSP) with jurisdiction over the place where applicant was born; and
- Applicant must submit PSA-issued late registered Certificate of Live Birth or Report of Birth.
If applicant is a Naturalized Filipino Citizen, the following are the additional requirements:
- For persons who have undergone Administrative Naturalization under Republic Act No. 9139:
- Certificate of Naturalization;
- Certificate of Renunciation of foreign citizenship issued by the concerned Foreign Embassy or Foreign Ministry; and
- Identification Certificate issued by BI.
- For persons who have undergone Judicial Naturalization under Commonwealth Act No. 473:
- Certificate of Naturalization;
- Court Order, Certificate of Finality of Decision granting naturalization, Decree of Naturalization; and
- Identification Certificate issued by BI.
- For persons who have undergone Legislative Naturalization under Commonwealth Act No. 63:
- Certificate of Naturalization;
- Certified true copy of the law granting citizenship;
- Foreign Birth Certificate authenticated by the Philippine Foreign Service Post; and
- Identification Certificate issued by BI.
- For persons who availed of the Facilitated Naturalization of Refugees and Stateless Persons under Supreme Court Rule 21-07-2022-SC:
- Decree of Naturalization
If applicant obtained Filipino citizenship by Election
- Applicants those born before 17 January 1973 to Filipino mothers and married to non-Filipino spouses and elected Philippine citizenship within three (3) years upon reaching the age of twenty-one (21), the following are the additional requirements:
- PSA-issued Certificate of Live Birth of or PSA-issued Report of Birth; and
- Additional documents proving election of Philippine citizenship.
If applicant retained or re-acquired their Philippine citizenship
- Applicants covered by the provisions of under Republic Act No. 9225, otherwise known as "Citizenship Retention and Reacquisition Act of 2003", shall submit any of the following documents issued by a Philippine FSP or by the Bureau of Immigration (BI):
- Order of Approval;
- Oath of Allegiance; or
- Identification Certificate or Certificate of Retention/Re-acquisition of Philippine Citizenship.
- Applicant is a derivative beneficiaries of Filipino parents who retained or re-acquire Philippine citizenship, shall submit the following documents:
- Order of Approval of parent or child (as applicable); and/or
- Identification Certificate issued by a Philippine FSP or Certificate of Retention/Re-acquisition of Philippine Citizenship issued by the BI.
BASIC REQUIREMENTS FOR MINOR RENEWAL APPLICATIONS (ELECTRONIC AND NON-ELECTRONIC PASSPORT)
- Confirmed Online Appointment;
- Accomplished Application Form;
- Personal Appearance of minor applicant and either parent or authorized adult companion;
- The accompanying adult companion shall submit supporting documents depending on the case of the minor. The list can be found below.
- Current ePassport with photocopy of data page;
- PSA-issued Certificate of Live Birth as proof of filiation;
- Valid Identification card or other competent proof of identity of the minor applicant; and
- Passport or valid identification card of the accompanying parent or authorized adult companion.
- Payment of fees: KD 19.500 (to be paid by applicant at the Embassy in person, in cash, Kuwait Dinar only)
Supporting Documents for Minor Renewal Applications
Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:

For renewal of a frequently Lost Passport
- Copy of latest-issued ePassport (if available);
- Police Report in English(filed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application);
- Notarized Affidavit of Loss in English (signed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application); and
- Amount to be paid: KWD 48.750.

For Damaged or Mutilated Passports
- Notarized Affidavit of Explanation (signed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application); and
- Amount to be paid: KWD 48.750.

For Marital Minor Applicants (with married parents as stated in the minor applicants's PSA Certificate of Live Birth)
- Valid Philippine passport or other identification card of the accompanying parent and the copy of his/her spouse’s passport or other valid identification card;
- If the accompanying parent is an alien, the non-Filipino parent must present his or her foreign passport and a copy of the Filipino parent’s Philippine passport or other valid identification card; and
- If accompanied by an authorized adult companion, a Special Power of Attorney (SPA) executed by either of the minor’s parents or his/her legal guardian, as the case may be, his/her passport or other valid identification card, and a copy of the passport or other valid identification card of the minor’s parents.
For Non-Marital Minor applicants (with unmarried parents as stated in the minor applicant's PSA Certificate of Live Birth)
- Only the mother applicant shall accompany the minor applicant in the passport application process;
- A Special Power of Attorney (SPA) executed by the mother will be required if she is not accompanying the minor applicant.
- If the Special Power of Attorney was executed abroad and not before an FSP, the Special Power of Attorney must be validly authenticated;
- Passport or Valid Identification card of authorized adult companion (List of Acceptable IDs for Passport Application).
In case applicant is not accompanied by the parent/s during the application process
- Applicant must submit a Special Power of Attorney executed by either of the minor’s parents or his/her legal guardian designating an adult companion to assist in the application process.
- If the Special Power of Attorney was executed abroad and not before an FSP, the Special Power of Attorney must be validly authenticated;
- Passport or Valid Government issued ID of authorized adult companion (List of Acceptable IDs for Passport Application); and
- Special Power of Attorney must have a copy of parent/s valid ID and/or passport attached.
For Non-Marital Minor Applicants with a Deceased Mother, but acknowledged by the Father (with unmarried parents as stated in the minor applicant's PSA Certificate of Live Birth)
- Personal Appearance of minor applicant and biological father;
- PSA-issued Certificate of Death or Report of Death of Mother;
- PSA Birth Certificate with Acknowledgement of Paternity (must indicate the name of the father in the birth certificate);
- Passport or Valid Government issued ID of biological father and legal guardian (List of Acceptable IDs for Passport Application)
In the absence of father, the following may accompany the minor child in the order indicated:
- Surviving grandparent (submission of PSA Certificate of Death of Mother;
- Oldest brother or sister, over twenty-one years of age (submission of PSA Certificate of Death of Mother and PSA Certificate of Death of the Grandparent);
- The child’s actual custodian, over twenty-one years of age (submission of court order for legal guardianship)
For Non-Marital Minor Applicants with a Deceased Mother and Father is unknown (with unmaried parents as stated in the minor applicant's PSA Certificate of Live Birth)
- Personal Appearance of minor applicant and court-appointed legal guardian

- Valid Passport or valid government ID* of adult guardian (List of Acceptable IDs for Passport Application)

In case none of the above can accompany the minor applicant
- In default of parents or a judicially appointed guardian, the persons exercising substitute parental authority under Article 216 of the Family Code of the Philippines shall prevail in determining the minor applicant’s companion in passport application, unless otherwise ordered by a competent Philippine court. In case of conflicting claims for custody, a court order shall be required; and
- In cases involving Filipino minors abandoned abroad, the Consular Official of the concerned Philippine FSP may initiate the passport application with the best interest of the minor being the primary consideration.
In case of Minor Foundlings in the Philippines not for Adoption
- Personal appearance of minor applicant and the NACC Social Worker or Liaison Officer;
- PSA-issued Certificate of Foundling;
- Letter of Guardianship issued by the Family Court; and
- Valid ID of the guardian.
In case of Minor Applicant who is a Prospective Adoptee for Domestic Adoption
- Personal appearance of minor applicant and NACC Social Worker or Liaison Officer;
- PSA-issued Certificate of Live Birth or Report of Birth or Certificate of Foundling;
- Valid passport or valid government-issued ID of authorized NACC Social Worker or Liaison Officer;
- Certificate of Child Available for Adoption or Deed of Voluntary Commitment / Certificate Declaring a Child Legally Available for Adoption issued by the National Authority for Child Care; and
- Official letter or Special Power of Attorney signed by the NACC Regional Office authorizing the Social Worker to process and receive the passport of the minor applicant and must include a copy of valid government-issued ID and/or passport.
Note: If an applicant is not accompanied by an authorized NACC Social Worker during the application process, the applicant must submit an SPA executed by the authorized NACC Regional Office designating the minor’s companion to assist in the application process and must include a copy of valid government-issued ID and/or passport.
In cases of Minor Applicant who is an Adoptee through Domestic Adoption
- Personal appearance of minor applicant and adoptive parent/s;
- Amended PSA-issued Certificate of Live Birth or Report of Birth, indicating the child’s adoptive name and name of adoptive parents;
- Passports or other competent proofs of identity of the minor applicant, adoptive parent/s or authorized adult companion; and
- Certified True Copy of Court Decree of Adoption or NACC-issued Order of Adoption.
In case of Minor Applicant who is a Prospective Adoptee for Foreign Adoption
- PSA-issued Certificate of Live Birth, Report of Birth or Certificate of Foundling;
- Endorsement from NACC;
- Certificate Declaring a Child Legally Available for Adoption or Deed of Voluntary Commitment;
- Placement Authority issued by NACC;

In case of Minor Applicant who is an Adoptee through Foreign Adoption

BASIC REQUIREMENTS FOR MINOR NEW APPLICATIONS

- The accompanying adult companion shall submit supporting documents depending on the case of the minor. The list can be found below.
- Philippine Statistics Authority (PSA) issued Certificate of Live Birth;
- Local Civil Registrar Copy may be required if PSA Certificate of Live Birth is not clear or cannot be read.
- Valid Identification Cards (at least one) of the minor applicant; and
- Passport or valid identification card of the accompanying parent or authorized adult companion.

Supporting Documents for Minor New Applications
Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:
For Newborn Applicants (less than 1 year old) and has No PSA Birth Certificate/Report of Birth yet
- If born in the Philippines - The minor applicant may submit a Certified True Copy (CTC) of Local Civil Registrar (LCR) Birth Certificate authenticated by the PSA.
- If born abroad - The minor applicant may submit original copy of Report of Birth filed at Post or first indorsement from Consular Records Division (CRD).
Additional Supporting Documents if the minor PSA-issued Certificate of Live Birth or PSA Report of Birth is late registered [one (1) year after the event]:
- School Records;
- Baby book or health record or its equivalent document;
- Foreign Passport (if applicable); and
- Residence card issued by host government (with english translation, if appliable).

- Notarized Special Power of Attorney (SPA) executed by either both of the parents or by the person exercising parental authority over the minor;
- Copy of the passport or other valid identification card of the minor's parents.
For Non-Marital Minor Applicant (with unmarried Parents as stated in the minor applicant’s PSA Certificate of Live Birth)

Important Reminders
- Personal appearance is required for all applicants.
- Incomplete requirements will not be processed.
- Earrings, necklaces, false eyelashes, facial piercings, and contact lenses are not allowed during data capturing. Smiling with visible teeth is also not allowed.
- Make sure to check the data in the computer monitor before affixing your signature to certify that the encoded information is accurate and correct. Your signature indicates your confirmation on the accuracy of the information contained therein. Once signed, any errors in the passport will no longer be the fault of the Embassy, which means the fee for the replacement of the passport will be shouldered by the applicant.
- Only immediate family members are allowed to get the passport on behalf of the applicant. Immediate family members include father, mother, brother, sister, spouse and children of legal age with proper authorization letter.
- Passport of a minor applicant shall be released to parents only or to an authorized representative with Special Power of Attorney.
- Passports unclaimed after six (6) months will be cancelled per Department Order No. 37-03.
- Please allow 4 to 6 weeks (45 days) waiting time after application for the release of your passport.
- For claiming of new passports, we require presentation of your old passport for cancellation and original official receipt before we can release your new passport. If your name is on list of passports for release, please bring your old passport and receipt when you pick up your new passport at the Embassy.
- Upon receipt, check all data in the new passport and affix signature on page 3.
- The Embassy may require additional supporting documents as may be necessary, especially for applicants with dual citizenship and with foreign-sounding family names to prove citizenship.
- Flight bookings should be made ONLY if passports are still valid for at least six (6) months beyond intended travel.
Republic Act No. 9225 or the Citizenship Retention and Re-acquisition Act of 2003 grants natural-born Filipinos who have lost their Filipino citizenship through naturalization in a foreign country the opportunity to retain or re-acquire their Filipino citizenship. Upon reacquiring their Philippine citizenship, they shall enjoy full civil, economic and political rights as Filipinos.
Kindly note that having dual citizenship (Filipino and foreign citizenship) is not automatic upon acquiring a foreign citizenship. A Filipino will lose their Filipino citizenship upon being naturalized as a foreigner, and will have to undergo the process below to reacquire/retain their Filipino citizenship.
This law shall equally apply to unmarried children below eighteen (18) years of age, whether legitimate, illegitimate, or adopted if they are included in the parent’s petition for reacquisition of Philippine citizenship.
RIGHTS AND PRIVILEGES
Once you reacquire your Philippine citizenship, you will again enjoy full civil, economic and political rights under existing Philippine laws.
Among these rights are:
- The right to travel with a Philippine passport;
- The right to own real property in the Philippines;
- The right to engage in business and commerce as a Filipino; and
- The right to practice one’s profession, provided that a license or permit to engage in such practice is obtained from the Professional Regulation Commission (PRC), or the Supreme Court in the case of lawyers.
You may also vote in Philippine national elections (for President, Vice President, Senators and sectoral representatives) by overseas voting in accordance with the provisions of the Overseas Absentee Voting Act of 2003.
You will enjoy all other rights and privileges enjoyed by Filipino citizens.
IMPLICATION ON INCOME TAXES
Under the Comprehensive Tax Reform Program of 1997, incomes earned abroad by Filipinos from 1998 onwards are no longer taxable. Hence, all Filipinos abroad, including those who have reacquired their Philippine citizenship, have been exempted by the Philippine Government from paying Philippine income tax on incomes earned abroad.
Incomes earned in the Philippines, however, will be subject to Philippine income tax.
RESIDENCY REQUIREMENT
Residency in the Philippines is NOT a requirement for those who reacquire Philippine citizenship.
VISA REQUIREMENT FOR FOREIGN SPOUSE AND/OR CHILDREN WHEN TRAVELING TO THE PHILIPPINES
As long as the foreign spouse and children travel with the Balikbayan, they will be entitled to a visa-free entry to the Philippines for a period of one (1) year.

Under Executive Order No. 408 and related issuances, the Department of Foreign Affairs allows nationals from eligible countries to enter the Philippines without a visa.
To avail of this privilege, travelers must present the following documents at the Philippine border control:
- A passport valid for at least six (6) months beyond the intended departure from the Philippines
- A return ticket to their home country or an onward ticket to their next destination
Nationals from 157 countries may enter the Philippines without a visa for tourism or business, with stays of 59, 30, 14, or 7 days, depending on their nationality and the Philippines' bilateral agreements with their country.

Citizens of countries not included in the list will need to apply for a visa prior to travel.
If you are a Kuwaiti passport-holder and wish to travel to the Philippines for tourism/business, you are eligible for the visa-free privilege provided that your stay in the Philippines does not exceed thirty (30) days.

VISA FREE COUNTRIES
- Nationals from the following countries are allowed to enter the Philippines without a visa for an initial stay of thirty (30) days:
- *Brazilian and Israeli nationals are allowed to enter the Philippines visa-free for a stay of fifty-nine (59) days based on existing agreements.
- Indian nationals may be granted a visa-free entry for an initial stay not exceeding fourteen (14) days under the Foreign Service Circular (FSC) No. 36-12, provided, that they possess:
- Valid or unexpired Japanese, Australian, Canadian, Schengen, Singapore, UK, or US visa or permanent residence permit;
- A national passport valid at least six (6) months beyond the contemplated stay;
- Return or onward ticket to the next country of destination;
- No derogatory record with the Bureau of Immigration, National Intelligence Coordinating Agency (NICA), and the International Police (Interpol).
The 14-day visa-free entry may be extended by an additional seven (7) days for a maximum twenty-one (21) days. - Under FSC No. 112-11, the following may enter the Philippines without a visa for an initial stay not exceeding seven (7) days provided that they possess a return or onward ticket:
- Holders of Hong Kong British passports; and
- Mainland Chinese for tourism with a valid or unexpired Australian, Japanese, Canadian, Schengen or US visa.
The 7-day visa-free entry may be extended by an additional fourteen (14) days to complete a maximum twenty-one (21) days.
Hong Kong SAR passport holders may enter the Philippines visa-free for an initial stay of fourteen (14) days.
